For grooming appointments, we need only the new client information and vet records.
- These forms include the new client information form, new dog information form
(please complete one for each dog if they have drastically different
personalities), our policies and procedures document, master agreement,
wellness plan and a credit card authorization form.
- Fax completed documentation to 713-583-7816
- Also ask your vet to fax your dog's vaccination history. To be on site at Deogi
Dog Daycare, Spa and Resort, we must have the following on site:
- Rabies certificate
- Proof of Distemper/Parvo within last three years
- Proof of Bordetella within last year
- Proof of negative fecal within last year
- Proof of use of flea preventative
